Case Management
Many people are in need of help when it comes to managing their day to day life and medical issues. Sometimes their loved ones are far away, or just not able to help out on a daily basis. Linda has experience helping people find the resources they need to accomplish their daily life goals. This can include in home assistance with activities of daily living, or finding a new living situation such as assisted living or a nursing home.
Linda will attend doctor appointments to make sure that the doctor has an accurate picture of how the client is functioning and then relay the information to the family or the client’s guardian.
If the client is unable to leave the home easily, Linda can arrange for medical care in the home at whatever level is needed and available.
If the client’s family is traveling and they need a local Medical Power of Attorney while they are gone Linda is available to fulfill this role on a temporary or extended basis.
